Plus you have to have to have a photo taken of you. Minors 15 and under are $60, + the $25 execution fee, and photo fee. Minors MUST HAVE BOTH PARENTS present to obtain a passport now, or proper documentation showing sole custody to one parent or a notarized form stating that a parent gives permission, etc. YOU MUST PROVIDE A CERTIFIED BIRTH CERTIFICATE, ISSUED WITHIN ONE YEAR OF BIRTH, SIGNED AND ISSUED BY THE REGISTAR./COUNTY. Passports take about 4-6 weeks to process or you can expedite this service for an additional $60 and receive your passport in 3 weeks. Good luck.
