This is redeemable against the price of your Fertility MOT. We would expect that the questionnaire will take about 30 minutes for you to complete, and you may be contacted by email or phone to clarify some of the details. The cost of having your blood samples taken at our centres is included in the price. If you are not having your bloods taken at one of our centres then you may incur an additional phlebotomy charge.
