I need help to diagnose someone who doesn’t report any hallucinations, but does have some very sophisticated delusions and elaborate rituals associated with those delusions. Outside of the delusions, rational thinking is high functioning and not impaired. There is social withdrawal, but it may be related to the delusions, or to the the depression, anxiety, and isolation the patient suffers as a result. There is a high likelihood of serious childhood trauma which means PTSD is also a possible factor, as well as some comorbid antisocial personality disorder signs. Help!
