Applicants are required to attach one set of self attested photocopies of all the required documents with the application form. In case of submission at Citizen Service Centre (CSC), particularly at Bangalore-1, applicants are requested to attach one set of attested photocopies (either gazetted official or notary) of all the documents with the application form. Original documents must also be produced at the counter; these will be returned after verification.
