For official paper CIS survey administration, the number of survey sheets that will be delivered is the same as the 12th class day enrollment as sent to the CIS system from the Office of Information Management and Analysis (IMA). For paper survey administration that is not for official CIS purposes, the number of survey sheets that will be delivered is the number of forms that are requested.
