The air vent needs to be in the chimney approx 1 foot (300mm) up from the floor.You only need 1no air brick in the chimney breast.But you dont say where the condensation is exactly.Are you sure its not damp on the external wall ?.If it is on the external wall you may have a bigger job than just fitting air bricks. Best you get a good local builder to have a look for you.Ask at your councils building inspectors dept if they will recommended you one you should find them very helpful.
