Your pack will be valid for 3 months. Please ensure you attend your eye test within that time. Please note that UCL is charged for your eye test once the pack has been issued to you. If, for whatever reason, you have not used your Eye Test Pack within the three month limit, please contact Occupational Health – do not apply for another pack.
