How long is the authorization valid?
The authorization is valid until its expiration date or the completion of a designated event. The maximum expiration date for an authorization form is two years from the date the form is signed. An authorization form can be revoked at any time. A revocation form can be requested from the NBC, or you can download the form from the Forms page.
