How long is a Commonwealth Care (Network Health Forward)plan effective?
In most cases, you are eligible for Commonwealth Care for one year, and you must renew your eligibility each year to keep your benefits. Every year, the Connector verifies your eligibility by mailing you a renewal form that you must fill out and send back. Your eligibility depends on your income, family size, employment, and disability status.
