To obtain an Application Packet for an Arizona Fingerprint Clearance Card, the candidate needs to contact the Arizona Department of Public Safety by phone (602-223-2279) to have a packet mailed. A fee of $47 is required and is payable by cashiers check or money order only. The candidate will need to contact the local police or sheriffs office for print rolling after receiving the application packet. After print rolling, a clearance card will be mailed back (this process if usually 10 weeks). Once a valid Fingerprint Clearance Card is received, the original Card, along with the Application for Certification, should be submitted to the Department of Education-Certification Unit. The Fingerprint Clearance Card will be returned.
