How is the financial limitation (therapy cap) applied in the Common Working File (CWF)?
Medicare shall apply these financial limitations in order, according to the dates when the claims were received. When limitations apply, the CWF tracks the limits. Contractors apply the financial limitations to the Medicare Physician Fee Schedule (MPFS) amount (or the amount charged if it is smaller) for therapy services for each beneficiary.
