The EPO plan is similar to an HMO. The EPO operates on a co-pay basis per type of care. There are no deductibles and no co-insurance. Participants of the EPO plan must use in network providers. Referrals are not required. The PPO plans operate on a co-pay (PPO III), deductible and co-insurance basis. Participants can choose to see an out-of-network provider at reduced coverage.
