The PCMH Survey includes: • Core items from the CAHPS Clinician & Group Survey. • Some CAHPS supplemental items and new items relevant to the medical home model. Users of the PCMH Survey can use the core items to calculate and report the same measures available from the Clinician & Group Survey.
