It is installed in the cement. Usually at the top four inches of the final pour. 4×4 square metal grid (think chicken wire but it’s more substantial) the tubing or pex (brand name) is zip tied down either in a serpentine or reverse return pattern. The later being more efficient. Concrete creates a very nice thermal mass for the radiant heat. Much superior to underpinning on a hardwood floor.
