You should name only one NHS Trust with which you hold a substantive or an honorary contract. Please ensure that if you do not also name this NHS Trust as your Institution in Section 2 Contact Details, that you include it and the position you hold there in Section 3 Summary of present and previous positions held. If you hold contracts with more than one NHS Trust you should still name only one NHS Trust on the application form. Substantive contracts should take precedence over honorary contracts.
