How is an orthodontic claim reimbursed with respect to what I will receive?
Once the initial claim has been submitted and the total benefit payable has been determined, United Concordia will issue 25% of the total benefit up front (for treatment plans started under the United Concordia plan) and the remaining benefit will be paid in equal monthly amounts over the remainder of the treatment plan. Qualified treatments in progress (treatment plans not started under the United Concordia plan) will receive a prorated amount in equal monthly payments over the remainder of the treatment plan.
