Arizona statutes and Arizona Department of Education regulations require that referrals must be initiated in a student’s public education agency, and must be approved by the local agency’s special education director. The NCR Cooperative referral form is called Request for Assistance, available in each school’s special education office, from any NCR teacher, by calling the NCR office at (928) 774-0655 or Contact Us. All referrals should include documentation of suspected hearing or visual impairment.
