Covered Persons will not be entitled to reimbursement for eligible medical expenses that are incurred as a result of an injury or sickness or a related injury or sickness for which the Covered Person has consulted with a physician or received any medical care, treatment or services, medication or prescriptions, during the six months before the effective date of coverage or the first day of the waiting period, whichever is earlier, until the following occurs: • After a Covered Person (employee or dependent), has been continuously enrolled in the Plan for twelve (12) consecutive months; • After a Late Enrollee (employee or dependent) has been continuously enrolled in the Plan for twelve (12) consecutive months.
