How have community agencies benefited from specialized geriatric services?
Our goal is to prevent disability and restore frail seniors to their highest possible level of independent functioning through service and collaboration with primary care physicians, community care access centers, and long-term care homes. We promote elder friendly communities, by responding to needs and building capacity. By coordinating the community-based, educational services of the psychogeriatric resource consultation program, the RGP provides an essential link in the region’s chain of psychogeriatric care providers across hospital, home and long-term care settings. This RGP’s new geriatric emergency management program is another example of services bridging the healthcare continuum.
