If eligible you may elect to be excluded from coverage by submitting to the Department of Industrial Accidents a completed, Form 153 “Affidavit of Exemption for Certain Corporate Officers or Directors”. For entities and individuals other than a corporation coverage for owners and members is excluded. You may elect coverage by submitting a written request on your letterhead indicating your name, status (owner, partner, and member) and desire to elect coverage.
