The Plan offers you medical benefits through a Preferred Provider Organization (PPO). Through this network, you have access to many participating physicians and hospitals. When you use in-network providers, you receive services at pre-negotiated discounted rates and at the higher network level of benefits. Youll have an annual deductible of $500 per person each calendar year and the Plan pays a percentage of covered expenses (coinsurance).
