NC-TOPPS employs two assessment forms on-line for data collection. The Initial Assessment (IA) is completed by clinicians when a client begins treatment. The Update Assessment (UA) is completed at scheduled intervals or when the client has a change in treatment status (e.g., discharge, transfer, etc.).
