Your elected annual goal amount is evenly distributed among the number of paychecks that you will receive. The funds will be deducted from your paycheck on a pre-tax basis. Once you have incurred an eligible expense, you simply download the reimbursement claim form located at http://www.miami.edu/benefits/forms, attach a copy of your receipt and mail the information to FBMC for a reimbursement.
