How does the lab detect penicillin resistance in pneumococci?
The routine screening test we use for detecting penicillin resistance in pneumococci is the Oxacillin disk test. This detects strains of pneumococci which have increased penicillin MICs. Strains fully sensitive to Oxacillin are fully sensitive to penicillin. Any strain that shows reduced sensitivity to Oxacillin has a battery of further antibiotic tests performed, including measuring the penicillin MIC. Pneumococci isolated from CSF or blood always have the full battery of tests performed immediately, without waiting for the Oxacillin results.
