How does the HHR affect Information Governance?
• Access to the HHR is tightly controlled; all parties submitting data and users of the system must adhere to a strict security and confidentiality framework (available on request). The security and confidentiality model which is used has been agreed by the LMC, BMA, Information Commissioners Office and the Caldicott Guardians from each of the PCT?s and Acute trusts and requires confirmation that the explicit consent of the patient has been sought and obtained before access to a record is permitted. A full audit trail is held, which is available to patients, and access by users is monitored to highlight any inappropriate access to medical records.
