How does the Hampshire Health Record (HHR) relate to the National Summary Care Record (SCR) and the local Care Record Service (CRS)?
• The HHR is a combined medical record populated with data from both primary and secondary care. This is provided for patients in the Hampshire area only. • The National Summary Care Record (SCR) is available nationally and contains much less information than the HHR. The local Care Records Service (CRS) is the system used mostly in hospitals that would enable sharing of clinical information to the SCR. • The HHR is a local project within Hampshire which is expected to eventually be replaced by the SCR when it is able to provide the full range of information currently available in the HHR.
