How does the Five Star Membership Program work?
Click here for the 5 Star Membership Form. Remember for every meeting, function, or service event you attend you must get your membership form signed by an Officer of Alpha Mu Tau. Officers should sign the day of the event and their signature certifies your participation on that date. Officers are not responsible for signing your membership form after the date of the event. If you fail to have an officer sign your form the day of the event, you may forfeit the points for that event. It is important that you keep track of your points. Once you have reached a Star-level, make a copy of your form and turn it in to the Student Life Office in the PTK mailbox. If you fail to submit your star-level form, you will not receive that level. You may submit your points towards the next level when you achieve that total.
