How does the business make a claim and receive its Settlement benefits?
The deadline to submit a Claim Form was June 20, 2008. The amount of the payment will be based on the amount of state USF surcharges paid by the business. Using data provided by MCI/Verizon, the Settlement Administrator will determine the dollar amount of state USF surcharges paid by each claimant and not subsequently refunded. The net settlement fund will be distributed among claimants in the same proportion that their state USF surcharges bear to the total of all state USF surcharges paid by all claimants.
