How does Sedgwick CMS get the physicians information for my STD claim?
If you are filing a new claim and we have not received information from your physician for this claim, tell your doctor to call Sedgwick CMS immediately. Your physician will need to supply Sedgwick CMS with medical information regarding your disability. If your physician is unable to call, you can download the Certification Form for Initial Certification of Disability for your physician to fill out. Fill out the top portion of the form and intstruct your physician to fill in the bottom portion of this form and fax it to Sedgwick CMS as soon as possible, to complete the filing process of the medical portion of your claim.
