Pletal is a quinolinone derivative that inhibits cellular phosphodiesterase. Cilostazol is a reversible selective inhibitor of phosphodiesterase, which suppresses cAMP degradation. It exhibits anti-platelet and antithrombotic effect by inhibiting both primary and secondary platelet aggregation without affecting bleeding time. Cilostazol also causes vasodilation by inhibiting calcium resultant contraction of smooth muscle cells, thus reducing plasma TG levels and increasing plasma HDL cholesterol levels. Cilostazol works by dilating arteries which supply blood to the legs. It also decreases the ability of platelets to stick together. A product developed by Otsuka America Pharmaceutical, Pletal quickly received FDA approval due to its beneficial work action and lower side effects.
