How does one obtain a permit to apply aldicarb (Temik) in Florida?
An Application for Permit to Apply Aldicarb (Temik) must be filed with the Department and approved before the aldicarb application is made. Submissions may be made online at http://www.flpesticidepermits.org/. Paper submissions should be submitted to the FDACS Pesticide Certification Section using the address on the form. To use the online permitting site, each individual must first register with the FDACS Pesticide Certification Section as an aldicarb applicator and obtain a username and password. A completed Request for Username and Password for Electronic Temik Permit Application may be faxed to the Pesticide Certification Section as indicated on the form, and a username and password will be provided by email. Permit applications can then be submitted online by logging into the permitting site at http://www.flpesticidepermits.org/.
