A completed Request for Username and Password for Electronic Temik Permit Application may be faxed to the Pesticide Certification Section as indicated on the form, and a username and password will be provided by email. Permit applications can then be submitted online by logging into the permitting site at http://www.flpesticidepermits.org/. A Users Guide to the online system may be obtained from the Pesticide Certification Section by calling (850) 488-3314 or sending an email to temik@doacs.
