How does one inform the office of a change of address?
A signed, written notice must be submitted to the Pesticide Certification Section to change any address on file. The notice must include the applicator’s complete name, license number, and new address, and the notice must be signed and dated. It is the licensed applicator’s responsibility to notify the Department of any address changes so important information will not be misdelivered or returned undeliverable. The fax number for the Pesticide Certification Section is (850) 922-6961. A change of information form may be used but is not required.
