A consultant firm must submit Part II – General Qualifications of the Standard Form 330 (Architect-Engineer Qualifications Form) along with a letter of Interest to Dan Peterson, Chief of Contract Administration, Vermont Agency of Transportation, National Life Building, Drawer 33, Montpelier, VT 05633-5001. VTrans requires the consultant firm to complete Part 2 of the SF330. The consultant may attach additional information regarding the firm. Once qualified, the firm will be added to our Qualified Consultant List for a period of two years.
