Contact and Address Information changes can be accomplished by completing the Address/Contact Information Change Request form, or by submitting a written request to the BCSE mailing address. Match Method and Media changes can be accomplished by completing the Contact Data and Program Option Sheet and submitting the request in writing to the BCSE mailing address. Financial Institution name changes or Federal Employer Identification Number changes must be accomplished by completing a new Financial Institution Data Match Agreement.
