It’s easy to enroll in FamilyBlue! You can complete the easy application online or over the phone – whichever you prefer. Once you receive your membership ID card, show your card at the time you receive service from a participating provider and you’ll receive the special FamilyBlue discount. There are no claims or forms to complete – you receive your discount on the spot.
