How does Crusader Community Healths sliding discount work?
As a Federally Qualified Health Center (FQHC), Crusader Community Health offers discounts based on federally established guidelines. You may be eligible for a discount if you are not covered by an insurance plan. Discounts are based upon family size and income. Proof of income is required to be considered for a discount. Discount levels are 25%, 50%, 75% and 100% on eligible services. There is a minimum $20 fee due per medical visit and a minimum $30 fee due per dental, optical, or podiatry visit, regardless of discount. Discounts are re-evaluated annually. Patients may be required to be assessed for eligible entitlements (entitlements may include Medicaid or other public benefits). Crusader Community Health provides assistance in completing entitlement applications. Most laboratory and radiology services for self-pay patients ordered and processed at the clinic are eligible for discounts.
