How does an approver know if a NOTE is attached to a document, and how does he/she access the NOTE from the Worklist?

If the user opens a document from the worklist, there is nothing to indicate that a NOTE exists. The document initiator should attach an e-mail indicating that the document has a NOTE. After the document is accepted into the system, the open item inquiry screens will indicate that a NOTE exists.
