To complete a change of address you will need FCC Form 605 (Main Form). The purpose of the filing would be Administrative Update “AU “. For a manual filing the appropriate form can be obtained via website: www.fcc.gov/formpage.html. To file electronically using the ULS you will need to utilize a Netscape Communicator Browser (i.e., 4.61; 4.75, 4.76, 4.5 version) with windows application.
