The parent may object to the immunizations on the grounds that it goes against their religious beliefs. If this is the case, the parent must sign the form at the link below, have it notarized, and provide it to the school. At the beginning of each year, the nurse should confirm that the parents still wish their child to be exempt. However, the parents do not need to redo the form each year.
