Providers receive a notification letter when the enrollment forms are approved and entered into the system. The letter is mailed to the business address listed on the application. Approval letters contain the new provider number, the effective date of enrollment and the address at which the services are provided. To check the status of enrollment forms sent to the Out-of-State Provider Unit, call (916) 636-1960.
