How does a candidate apply for an Arizona Substitute Teacher Certificate?
The Substitute certificate requires a bachelors degree from an accredited institution and a Valid Fingerprint Clearance Card. First, obtain the fingerprint clearance card (see #5 above). Mail the Fingerprint Clearance Card (original), application for certificate (www.ade.az.gov/certification/certforms.asp), official transcripts* with degree posted, and a $60.00 cashiers check, money order or personal check to Arizona Department of Education Certification Unit, P. O. Box 6490, Phoenix, AZ 85005-6490 or bring it to the Certification Unit at 1535 W. Jefferson, Phoenix, AZ. (*An official transcript is a record of courses taken from a university or college bearing the official seal and signature of the registrar of the institution. Transcripts are not returned.
