Never use PVA under paint. Remove paint by stripping and sanding until you get back to a solid surface and this is one of those rare occasons where a primer like Bullseye 1-2-3 might be used first. I would not recoment ordinary emulsion in a bathroom especially if you are having problems, but use Dulux quick drying eggshell.

Difficult as the paint is always going to flake if it isn’t bonded to the previous coat. It was probably applied over dirt and grease. You could try a coat of diluted PVA. This should seal it to an extent. The best bet would be to scrape it all off if you can. Clean the ceiling well, and repaint with a bathroom quality emulsion.
