If you’d rather turn them in and claim their value, you can do that, too. Read on to learn more. Contact the institute where you purchased the bond if it was ordered and not received after 15 days. Narrow down the issue date of the bond if you don’t know the exact date it was issued. Fill in the issue date, or a range of dates you can narrow the issue down to. Download a Form PD F 1048 from the Treasury Direct Website, or request that the form be mailed to you. Choose a replacement bond or payment for the bond when sending in the replacement form. Fill in the bond amount and bond number. Complete the form including the social security number, names and address on the bond. The bond replacement form must be signed by a certifying officer.
