In addition, your parent, legal guardian or legal representative may also request a copy of your birth certificate. You can request certificates in person at one of the state’s Bureau of Vital Statistics offices (located in Anchorage, Fairbanks and Juneau), by fax, or by mailing your request to the state office in Juneau (see Resources). Visit the Alaska’s Bureau of Vital Statistics website (see Resources) and download the Alaska Birth Certificate Request Form. The form is a PDF document, so you’ll need to have Adobe Acrobat Reader 5.0 or higher on your computer. Print the form and compete all the appropriate fields. Remember to sign and date the form and include all pertinent information. Use your full first, middle and last names as they appeared on the original birth certificate.
