How Do You Obtain An Alaska Birth Certificate?
If you need to obtain a certified copy of an Alaska birth certificate, you can do so via mail, fax, internet or in person. The Alaska Bureau of Vital Statistics handles birth certificate requests for births that took place within the state. You must be eligible to request a birth certificate in Alaska; specifically, you can only request your own record or your child’s. Otherwise, you must present documentation showing that you are authorized to request the birth certificate. Locate and complete the application request form for an Alaska birth certificate (see first link under References). Fill out the form using your computer and then print it out afterward. Or, print out the form first and complete it by hand. The form asks the child’s full name, birth date and location, mother’s full maiden name, father’s full name and the reason for the request.
