How do you log the discharge letter when it comes from a different hospital to the one the patient was originally refered to?

When the discharge letter comes back and they’ve been somewhere else, then what we can do is edit the original referral to the new hospital. When creating the additional referral we now just put the new hospital in as to where they are going.
