The first one you stop by should give you an immunization card free of charge. Get each doctor office that has the records add the shot record to the same immunization card if possible. Locate the immunization registry for your state if you are unable to locate one or more of your immunization records. You should be able to do a quick Google and find it. These registries have been around a little over a decade, so it can be possible that a teenagers records may not be able to be located.
