How Do You Find Lost Immunization Records?
Immunization records can be a pain to deal with when they are lost, especially if you have visited more than one doctor’s office. Gather all the phone numbers and addresses to the past doctors your child has visited for check ups or vaccines. Call each doctor’s office to see if your child’s records are there, though you will get very limited information over the phone. Go down to the offices/clinics one at a time. The first one you stop by should give you an immunization card free of charge. Get each doctor office that has the records add the shot record to the same immunization card if possible. Locate the immunization registry for your state if you are unable to locate one or more of your immunization records. You should be able to do a quick Google and find it. These registries have been around a little over a decade, so it can be possible that a teenagers records may not be able to be located.
