You must file this form within 90 days of filing your Articles of Incorporation and then re-file it biennially after that for most non-profits (annually for credit unions and consumer cooperative corporations). Obtain a copy of Form SI-100. You can pick this up from your local Secretary of State’s office or download it from their website (see Resources).
