Changing your name with the Social Security Administration and the Department of Motor Vehicles are the first steps. Order a certified copy of your marriage certificate from the local courthouse in the area where you were married. Apply for a name change with the Social Security Administration to have your new name linked to your Social Security number. To apply for a Social Security name change, fill out form SS-5.
