When someone has altered mental status, it is important to assess their mental status and reassess the mental status every once in awhile to assure that the patient is not getting any worse. Assess and document eye responses. The highest score that they can get for eyes is a 4, in which the patient opens their eyes spontaneously. If they open their eyes in response to voice, score them a 3. If they open their eyes in response to painful stimuli, give them a 2. If they do not open their eyes at all, despite all the above attempts, score them a 1, the lowest score they can recieve. Assess and document the patient’s verbal response.
